Position: Transitional Housing Case Manager (SPOA Intake)
Division: CHOICES Mental Health Housing
Program: Intake
General Description: Supervision and management of Intake Department, responsible for promoting services to referral sources, processing and assessment of referrals, coordinating admissions, and resident aftercare.
Qualifications: Bachelor’s Degree preferred in a Human Service related field and 1 year of Human Service experience. Clean and valid NYS Driver’s License.
Salary Range: $38,355 - $42,764 annually Hours: Full Time
