We are seeking a(an) LPN Inpatient Rehab for our team to ensure that we continue to provide all patients with high quality, efficient care. Did you get into our industry for these reasons? Job Summary and Qualifications

  • You will collect health information per interviewing, observing, and measuring vital signs and specimen collections.

  • You will monitor patients’ safety, symptoms, changes in behavior and/or condition, IV sites, and medication side effects and effectiveness

  • You will report unsuccessful interventions and communicate patient’s needs to RN, physician, social services, Physical/Occupational therapy, and Care Management staff.

  • You will make suggestions to improve and individualize care. You will administer specified medication accurately and in accordance with hospital and regulatory agencies’ policies and legal limitations.

  • You will assist patients with Activities of Daily Life

  • You will provide information to patients and their families regarding the unit, medications, procedures, and discharge plans.

Qualifications:

  • This position requires a level of knowledge which is acquired through education preparation in an accredited LPN program.

  • Current NH LPN license or compact state.

  • Working knowledge of underlying applicable principles, policies and procedures for the following entities: CDC, CMS, ANA, NH State Board of Nursing (Nurse Practice Act)

  • IV Therapy Certification required.
